(AP) -- DES MOINES, Iowa - State officials have levied $11,500 in fines against a home for disabled people in connection with the death of a 26-year-old man there in November.
According to a copyright story in Monday's Des Moines Register, Timothy Alexander began suffering seizures on Nov. 19 at Glenwood while eating dinner. State inspectors say after the seizures began, a series of delays, errors, miscommunications and problems with equipment ensued.
According to the Register, the fines stem from an alleged series of errors by caregivers.
Iowa Department of Human Services spokesman Roger Munns says after an internal review the department has taken "personnel action" and is also addressing "systemic issues."
